EPA Region 4 is piloting a Sole Source Aquifer (SSA) project review form. Please note that this form is only applicable to EPA Region 4 states of Alabama, Florida, Georgia, Kentucky, Mississippi, North Carolina, South Carolina, and Tennessee. If your project is located outside of an EPA Region 4 state please contact the regional contact for the state where your project is located at the following link. https://www.epa.gov/dwssa/sole-source-aquifer-contacts-epas-regional-offices.
