Lead Consumer Notice of Tap Sample Results

The Lead Consumer Notice of Tap Sample Results template can be used by water systems in Wyoming or on Tribal Lands in Region 8. The form is used to notify customers of the lead results in the drinking water where samples were collected.

Systems are required to send lead results to consumers within 30 days of receiving the test results from the laboratory.

Systems are also required to submit one copy of this form, along with the Lead Consumer Notice Certification Form to EPA, no later than 90 days after the end of your monitoring period. 40 CFR 141.85(d)
