Fumigant Management Plan Templates - Phase 2 Files Listed by Chemical
EPA created templates in PDF and Word formats for the management plan for each soil fumigant.
- The PDF templates are for users to print and fill out by hand.
- The Word templates are for users to fill out on the computer.
The agency also created samples for each chemical of filled-out:
- Fumigant management plans (FMP).
- Post-Application Summary reports.
- Related forms.
The latter two items are included in the fumigant management plan sample.
Since label requirements vary based on the soil fumigant, please use the appropriate chemical-specific templates. Note that phase 2 labels can be identified by the presence of a "Terms used in this labeling" section.
